The following facility has merged with the adjacent facility. Current facility address is 21739 Stevens Creek Blvd, merging with 21741 Stevens Creek Blvd. Any addition of equipment including but not limited to new equipment, refrigeration, and plumbing will require plan submittal and approval through plan check.

In the facility, observed employee food and equipment stored at the 3 compartment sink and in amongst the facility storage. [CA] Ensure employee food, equipment, and personal items are stored separately to prevent possible contamination.

At the handwash station, observed equipment stored in the basin. Per operator, handwash station is leaking and requires repairs. Facility is currently sampling food and requires an accessible handwash station. [CA] Ensure handwash station is accessible at all times and provided with hand soap and paper towels to allow for proper handwashing. [SA] Operator to temporarily use the first right compartment of the 3 compartment sink for handwashing until the handwash station may be repaired.

In a freezer chest, observed various frozen dumplings and buns stored without proper labeling. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

Food facility is sampling open food products without a food safety manager's certificate. [CA] Facilities that provide any open food preparation shall provide employee training for food safety. At least one food safety manager's certificate shall be obtained and available within the facility for review. All other employees that handle food shall obtain food handler cards.
