This was a follow-up to the 6/23/25 routine inspection. The major violations were observed to be in compliance as follows: 1) The paper towels and soap were stocked in both the men's and women's restrooms. 2) No temperature violations were observed (no foods were stored inside the countertop glass door refrigerator). 3) No evidence of cockroaches was observed. The yellow placard was replaced with a green placard. OK to operate. Note: PIC provided pest control report dated 7/7/25 stating no activity was found. PIC stated facility will continue to receive monthly pest control services. PIC stated that bags of ice were placed inside the glass door refrigerator to help reduce the ambient temperature. Continue to closely monitor the food temperatures to ensure they stay at 41F and below; an ice bath would be another option.

    Observed one live nymph cockroach in a sticky trap on the floor under the kitchen handsink. No other evidence of vermin was observed. [CA] Keep facility clear of vermin. [SA] The sticky trap was removed from the facility.

    In the customer self-service area: observed apples were stored without a sneeze guard/etc.. There was no sign stating the apples needed to be washed before consumption. [CA] Protect fruits with edible peels from contamination (via sneeze guards/tongs, wrapping fruits, etc).

    The quaternary ammonia test strips were expired. [CA] Obtain non-expired sanitizer test strips.

    In the customer self-service area, inside the glass door countertop refrigerator: measured internal temperature of milk to be 53F. Operator stated the milk inside the refrigerator had been held since 8am today, and yogurt had been held since 7am today (both less than 3.5 hours prior). Ambient temperature of unit measured 53F with an IR thermometer. [CA] Keep potentially hazardous foods at 41F/45F and below. [SA] Foods were moved to a different refrigerator.

    In the men's restroom: observed the soap and paper towel dispensers were empty. [CA] Keep soap and paper towel dispensers filled at all times. [COS] Dispensers were refilled. _______ Minor violation: At the kitchen handsink, observed the wall-mounted soap dispenser was not dispensing adequate amount of soap. Per operator, the battery was low. [CA] Keep dispenser in good working condition at all times. [COS] The batteries were replaced and the dispenser was observed to be working properly.

    The countertop glass door refrigerator in the customer self-service area was not able to keep potentially hazardous foods at 41F/45F and below. Ambient temperature of unit measured 53F. [CA] Repair/adjust the refrigerator so that it can keep foods at 41F/45F and below.

    Under the ice machine and prep sink: observed drain pipes extended into the floor sinks. [CA] Maintain a minimum one inch air gap between the drain pipes and the floor sink rims.
