Hand wash station in the back room obstructed with large floor fan placed in front. [CA] Handwashing facility shall be clean, unobstructed, and accessible at all times.

FOLLOW-UP INSPECTION

(1) 1st floor outside bar. This was a follow-up to assess whether the condition regarding the restriction of the BIB dispensing system could be lifted. Documentation had been provided to DEH from the BIB manufacturer (PEPSICO) confirming that the equipment was safe for use. Another site walkthrough was conducted today to verify any affected areas were still in safe condition. The BIB system-restricting condition is lifted and the facility may now use the BIB dispensing system for their bars and other beverage dispensers. Due to technical difficulties, this report will be emailed to the facility. Joint inspection with Aziz Hussain.

FOLLOW-UP INSPECTION

1st floor outside bar. This was a follow-up to the 5/19/25 inspections regarding a sewage backup incident that occurred 5/18/25 (See CO0156878). - The facility had submitted via email written documentation of how the affected areas and equipment were properly cleaned and sanitized with ECOLAB OASIS MULTI-QUAT SANITIZER, 200PPM last night (all floors and equipment in kitchens, bars, restrooms, beverage prep rooms, hallways, furniture). - During today's site visit, the previously affected areas (kitchens, restrooms, bar area, beverage prep rooms, BIB room) were inspected. No evidence of contamination or sewage backup was observed. - The previously closed facilities (1st and 2nd floor kitchens, 1st floor bar) were re-opened and are permitted to operate. ***All bars/beverage rooms are conditionally allowed to operate, but only prepackaged beverages are permitted. The facility is not permitted to use any drink/beverage dispensing equipment until they provide this Agency a statement from the BIB vendor/manufacturer confirming that no contamination could have been inside the pipes/tubing/beverage containment, that the equipment can prevent seepage even after being in contact with sanitizer, and that line replacement is not required.*** DEH will follow-up with a request for a meeting with the facility manager and a letter will be provided in the coming days. FACILITY PERMITTED TO RESUME FOOD AND BAR OPERATIONS UNDER THE CONDITION OUTLINED ABOVE. Consulted supervisor M. Huato. Due to technical difficulties, this report will be emailed to the facility.

ROUTINE INSPECTION

1st floor outside bar report. Conducted a site investigation today, May 19, 2025. Per PIC Jim, Director of Operations, there was a backup at 8:30pm last night. Plumbers were called and unclogged the line, but wastewater affected multiple areas, including parts of kitchen areas and the beverage storage room which supplies beverages to all bars and beverage stations. A remediation company (BELFOR) then came and cleaned and sanitized all affected areas. However, the disinfectant type (BENEFECT BOTANICAL DISINFECTANT) could not be verified as approved. Therefore, all food facilities are closed and bars can only serve prepackaged drinks (nothing from the bib/lines), EXCEPT FOR THE 1ST FLOOR INSIDE BAR which must close completely. When all affected areas (kitchen, bars, floors) have been sanitized and documentation can be provided for: 1) Type of sanitizer 2) Cleaning/sanitizing procedures 3) Concentration of Sanitizer 4) Which equipment was cleaned/sanitized and how (inside and outside surfaces), the facility may call (408) 918-3400 to schedule a reopening. Due to technical difficulties, this report will be emailed to the facility.
