FOLLOW-UP INSPECTION

334 Commercial St. Follow Up This was a follow-up for the kitchen at 334 Commercial St. No live cockroaches observed. Two deceased cockroaches observed on the floor near prep tables near the wall with the handwash sink. No other evidence of vermin observed. Per PIC, pest control serviced the facility on Saturday, October 5. Please email the pest control reports to mindy.nguyen@deh.sccgov.org when they are available.

334 Routine Note: - A follow up inspection is scheduled for Monday 10/7/24 to assess compliances for violation K23 above. The first follow-up is free of charge. Any additional follow-up inspection(s) shall each be billed $298/hr during business hours. Failure to comply may result in enforcement action. Please call Mindy Nguyen at (408) 918-3490 if there are any changes or updates. - Per PIC, pest control services facility twice a month. Recommend increasing frequency to at least once a week for at least a month. OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: IKITCHENS NEW OWNER: Esha Corp The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP07. An invoice for the permit fee in the amount of $968.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 11/01/24 - 10/31/25. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 10/03/24. *Permit condition: Facility is permitted as a rental kitchen and shall ensure all food operations at this facility have valid environmental health permits and other required licenses/registrations for their operation.
