OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Stickhouse NEW OWNER: The Corner, LLC. The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP13. An invoice for the permit fee in the amount of $1,148.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2026 - 06/31/2026 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 06/10/2025 *Permit condition: Facility shares kitchen and various equipment with adjacent facilities; FA0300693 and FA0300695. Use of the approved kitchen is required at all times to operate. In the event that an imminent health hazard is observed at either facility, operations at all three locations may be subjected to immediate suspension until the hazard is adequately addressed and corrective actions are verified by this Division. ** Note: - Per owner, concept of the facility is still in the planning stages: - A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling. This includes installation of equipment not already approved during the facility evaluation inspection and on the structural review report. - Facility plans on preparing gelato as one of the menu items. - A milk product plant license will be required from California Department of Food and Agriculture (CDFA). - A person shall not engage in the business of dealing in, receiving, manufacturing, freezing, or processing milk, or any product of milk unless a license or permit has been obtained from CDFA for each separate milk product plant or place of business.
