Chick'N Out Routine inspection OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: CHICK'N OUT NEW OWNER: CHICK'N OUT LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. Note: owner provided seller's permit showing the new ownership change occurred in April 2025. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,383.75 ($1,107.00 + $276.75 25% penalty fee for operating without an Environmental Health permit) will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 07/01/2025 - 06/30/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 6/24/25. *Permit condition: 1. ON SITE CONSUMPTION OF FOOD IS NOT ALLOWED BASED ON SUBMITTED DESIGN 2. APPROVAL OF THE MOFFET CONVECTION OVEN WITHOUT AN EXHAUST HOOD IS CONDITIONAL AND LIMITED TO ONE UNIT AND BAKED GOODS ONLY. IF THERE IS A FUTURE CHANGE IN MENU OR OPERATION, OR THE USE OF THIS EQUIPMENT CAUSES AND SANITATION, VENTILATION, OR SAFETY PROBLEM, THE EXHAUST HOOD EXEMPTION MAY BE REVOKED AND APPROVED MECHANICAL EXHAUST SYSTEM MUST BE INSTALLED OR THE UNIT REMOVED. VENTILATION PROBLEMS INCLUDE BUT ARE NOT LIMITED TO, EXCESS HEAT IN THE FACILITY THAT RESULTS IN UNCOMFORTABLE WORK CONDITIONS FOR EMPLOYEES. THE EXEMPTION SHALL NOT BE DEEMED TO SPERSEDED ANY LOCAL BUILDING AND FIRE CODE REQUIREMENTS. IN THE EVENT THAT A FUTURE CHANGE OF OWNERSHIP OCCURS, THE NEW OWNER SHALL BE MADE AWARE OF THESE CONDITIONS. *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date.     Observed large flour bin stored in front of the kitchen handsink, partially obstructing it. [CA] Do not store objects in front of the handsink. Handsinks must be accessible so that handwashing is not only possible, but likely.

    1) Observed a knife was on the floor propping the restroom door open. 2) Observed pink mold-like substance on the white baffle inside the ice machine. [CA] Keep food contact surfaces clean and sanitized.
