[CA] Store contaminated dishes in designated wash compartment of the 3-compartment sink. The sanitizing compartment and drainboard of the 3-compartment sink shall be maintained in clean and sanitary condition.

FOLLOW-UP INSPECTION

On-site to conduct a follow-up to the routine inspection and closure on 8/13/2025. The following major violation has been corrected: - K23: No live vermin activity observed during inspection. 1. Checklist for re-opening has been received from Nelio and Teresa Defreitas. 2. Reports from a licensed pest control company has been received. Reviewed services from 8/13/2025 and 8/14/2025 from Killroy. 3. All sanitation and food safety items on worksheet have been verified as completed. Facility has conducted thorough cleaning in the kitchen area and in the 3-compartment sink, where the majority of the infestation was located. The grease trap cover has been replaced and sealed, and the gaps in the back door have been sealed with foam to prevent pest entrance. 4. All prevention measures on worksheet have been verified as completed. Facility will continue to receive routine pest control services from Killroy monthly. The facility is hereby authorized to reopen by this department. Continue to address remaining violations as cited on the routine inspection report.

ROUTINE INSPECTION

The facility is hereby closed due to evidence of vermin infestation (K23). The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider. No pest control reports available for review. Per PIC, facility received services 3 weeks ago. - The first follow-up inspection is free of charge. Subsequent follow-up inspections after the first follow-up shall be billed at $298/hour during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. - A facility found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Notification: The person in charge during inspection, Nelio Defreitas, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food.
