Do not over stacked potentially hazardous foods over the fill line when using the prep unit insert. [COS] Employee relocated the over stacked eggs into the walk-in cooler.

[COS] Employee was observed diluting sanitizer from dispenser with water. Discussed and worked with employee that the solution coming from the dispenser is already mixed and diluted to the appropriate concentration.

The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 08/01/25 - 07/31/26 This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. An official permit will be mailed to the address on file and shall be posted in public view upon receipt. *Structural Review inspection conducted on 07/28/2025 *Obtain food safety manager certificate within 60 days. All other food employees must have valid food handler cards within 30 days from hire date. * Any changes made to the kitchen such as the modification or addition of cooking equipment or equipment with plumbing are required to be submitted to this department's Plan Check for review and approval prior to installation and use. ROUTINE INSPECTION

Facility changed their facility name from Rocket Space Burger back to Von's Chicken. Business license and current health permit are operating under the dba: Von's Chicken. Confirmed that there is no ownership change. Menu also changed back to what was previously approved and burgers has been removed from the menu. The following was discussed with employees and PIC: - Reminder that equipment or utensils that fall onto the ground shall be properly wash, rinsed, and sanitized. - Proper storage of utensils that are in active use.

    Employee observed handling raw chicken with their bare hands then wiped hands using a towel and proceeded to put on disposable gloves to handle food. [CA] After handling raw foods, employees shall properly wash hands using warm water, soap, and single-use paper towels prior to putting on disposable gloves or handling other foods. [COS] Employee was stopped and told to properly wash hands. Employee removed the towel used to wipe hands.
