    Observed three dead cockroaches on the outside door into the kitchen from the restroom corridors. Employee stated that facility is treated every two to three months. Latest pest control report unavailable for review. No other signs of live or dead activity was observed. [CA] Clean and sanitize area of dead cockroaches. Observed numerous fruit flies in the facility, especially in and around the trash can outside of the restroom. [CA] Food facility shall be kept free of non-disease carrying insects, weevils, ants, gnats, and fruit flies.

  • K36: Equipment, utensils, linens: Proper storage and use

    Observed employee utilizing three-compartment sink for food preparation when a food preparation sink was available for use. [CA] Utilize sinks for their designated purpose. All food preparation shall be conducted at the food preparation sink. Facility utilizing cloth linen to cover food (washed vegetables, herbs, pickled carrots and daikon). Observed some cloth with black mold-like growth all around. [CA] Linen shall not be used in contact with food unless they are used to line a container for the service of foods and are replaced each time the container is refilled for a new consumer and laundered prior to reuse.

    Bags of frozen shrimp thawing in ambient temperature in the middle basin of the three-compartment sink. [CA] Frozen potentially hazardous food shall only be thawed in one of the following ways: 1) under refrigeration that maintains the food temperature at 41°F or below, 2) completely submerged under potable running water for a period not to exceed two hours at a water temperature of 70°F or below, and with sufficient water velocity to agitate and flush off loose particles into the sink drain, 3) in a microwave oven if immediately followed by immediate preparation, 4) as part of a cooking process.

    Hand soap dispensers at both hand wash station in the kitchen were inoperable. Third hand wash station in the front service area obstructed with blender inside the wash basin. [CA] Provide handwashing cleanser in dispenser at handwash stations at all times. Paper towels and hand soap shall be readily available and easily accessible at all times. [COS] Hand soap was provided at hand wash stations.
