Notes: - Per PIC, they sent in staff Food Handler Cards to Levy for participation at Levi's. - Upright dishwasher passed with use of thermolabels twice, but reached a maximum of 156.5 with DishTemp on multiple attempts. Dishwasher technician onsite stated the dishwasher needs the water equalizer cleaned out/changed and would do it tomorrow. Facility to manually wash, rinse, and sanitize. DishTemp will have calibration tested before next use.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Scratch Cookery, Inc NEW OWNER: Scratch Cookery, Inc The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 12/01/2024 - 11/30/2025 This report serves as a temporary permit. In the upright hot holding unit, cooked chicken measured between 127-130°F. Per staff the chicken had about 45 minutes since being cooked. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Staff moved the fries to the walk in cooler to rapidly cool. Staff agreed to discard the chicken in the warming unit after 4 hours and marked it for discard. The unit was adjusted to increase ambient temperature as it was at 105°F.
