OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: Healthy Habits NEW OWNER: Kalin Esavi The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP09. An invoice for the permit fee in the amount of $815.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. Facility serves food from that is prepackaged and individually packaged for purchase from Healthy Habits Meal Prep with a commissary at iKitchens in San Jose. PIC is submitting the application to complete the change of ownership, must be submitted by 10/17/2025.

Reference CO0155407 - Facility is hereby closed due to failure to supply adequate hot water. - Facility is to remain closed until: Hot water at a minimum 120F is supplied to sinks in food facility (except hand wash sink, must be minimum 100F). - Facility is to remain closed until authorization is given by this Division to reopen. - Contact district inspector once major violation has been corrected and checklist items have been completed at 408-918-3480 or the main office at 408-918-3400. - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied.
