ROUTINE INSPECTION
This facility shall remain closed until all major violations have been verified corrected by this department. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale. Subsequent follow-up inspection after first follow-up shall be billed $282/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. If this facility is found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.
Due to the associated facilities common permit being temporarily suspended, warewashing facilities are currently inaccessible for this facility. [CA] Warewashing facilities shall be accessible at all times.
ROUTINE INSPECTION
Used wiping towel is observed stored on prep surface of food prep unit located in the back kitchen. [CA] Wiping towels shall be stored in sanitizing solution of an approved concentration when not in use (100 ppm - chlorine, 200 ppm - quaternary ammonium).
Small flies observed in the back prep area. [CA] Food facility shall be kept free of flies and fly activity.

Notes: - If "egg" add-on is offered undercooked, ensure the consumer advisory indicates it is undercooked. - Ensure the current Environmental Health Permit is posted in public view. - Have all food handler cards store together for review.
At the front handwash sink: observed the liquid soap dispenser dispensed very small amounts of soap at a time. Observed the lid for the soap dispenser was missing. [CA] Repair dispenser and replace missing lid.
