ROUTINE INSPECTION
This facility shall remain closed until all major violations have been verified corrected by this department. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale. Subsequent follow-up inspection after first follow-up shall be billed $282/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645 for a minimum of two hours, during non-business hours, and upon inspector availability. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. If this facility is found to be in continued operation without a permit may incur a penalty of three (3) times the operating permit fee, and fee(s) for re-inspection(s) charged at the current hourly rate approved by the Santa Clara County Board of Supervisors. Penalties and fees incurred due to enforcement activities must be paid in full prior to the issuance of an operating permit.

ROUTINE INSPECTION
In the warewash area, observed fruit flies. [CA] Ensure facility is kept free of fruit flies to prevent possible contamination of food.
At the front counter, measured pepperoni pizza at 98F. Per operator, pizza is stored for no more than 4 hours from 12:00pm to 4:00pm. Time log was not filled out. [CA] Potentially hazardous foods maintained by time shall be time labeled/time marked when removed from temperature control and served within 4 hours or discard. [SA] Operator to serve pepperoni pizza by 4:00pm or discard for a total of 4 hours from when removed from temperature control.
ROUTINE INSPECTION
Discussed and signed TPHC form during inspection.
Observed soiled wiping cloths stored on food preparation areas. [CA] Wiping cloths shall be stored in sanitizing solution of an approved concentration when not in use.
Per PIC, cooked pizzas at the service line are held on time as a public health control (TPHC). At the time of inspection, observed lack of time markings. No written procedures are available for review. [CA] When time only, rather than time and temperature is used as a public health control, PHFs shall be time marked to indicate when item is removed from temperature control, discarded if not consumed or served within 4 hours, and written procedures shall be readily available for review. [COS] PIC wrote time markings for the pizza and completed TPHC form during inspection.

The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 10/01/2024 - 09/30/25. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
