Notes: - Facility shall not repackage foods as there is no dedicated handwash sink in the facility. Hand washing at the restroom handwash sink is not acceptable for food preparation. - Facility shall submit plans for the addition of the walk in cooler.

- K43: Toilet facilities: properly constructed, supplied, cleaned
Repeat Violation: The restroom door does not self-close. [CA] Toilet rooms shall be separated by well-fitted, self-closing doors that prevent the passage of flies, dust, or odors.
- K32: Food properly labeled and honestly presented
Repeat Violation: Facility has repackaged foods (beans) that do not bear a label. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

A rotting onion was at the produce sales area. [CA] Moldy food shall be removed immediately from use. [COS] PIC removed the onion from the sales area.
- K44: Premises clean, in good repair; Personal/chemical storage; Adequate vermin-proofing
There is a gap that is larger than 1/4' inch under the exit door of the facility. [CA] Eliminate gap greater than 1/4" to prevent the entrance and harborage of vermin.
- K48: Plan review
Facility is in the process of installing a new walk in cooler. Facility has also removed interior walls separating the sales area and the back storage room. Exposed electrical conduit was observed on the ceiling. [CA] A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling.
