    Five-year Food Safety Certificate (FSC/food manager certificate) unavailable for review. Employee's also lack three-year food handler cards. [CA] Food facilities that prepare, handle, or serve non-prepackaged food shall have a valid Food Safety Certificate available for review at all times.

    Observed employee eating in the back area where opened food is packaged. [CA] No employee shall eat or drink in the food prep, food contact storage, or food storage areas, use designated area.

    Frozen prepackaged food items do not have any labels. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

    Hot water at the three-compartment sink measured at 119F max. [CA] Hot water shall be supplied at a minimum temperature of at least 120°F measured from the faucet throughout facility except at handwash sinks.

    One dead cockroach observed on floor behind the merchandiser refrigerator in the back. PIC stated that facility is serviced monthly, and that a new company was just contracted. No other live or dead activity was observed. [CA] Clean and sanitize area of dead cockroaches or old droppings.

  • K48: Plan review

    Facility has conducted the following remodeling of the facility without prior approval: - Relocation of the water heater from the backroom into the restroom. - Previously approved location of the hand wash station has been relocated to in front of the janitorial sink. - Bar style three-compartment sink was installed in area where the water heater and hand wash station was previously approved. - Facility has added an extension to the existing building for storage of fruits and two chest freezers. [CA] A person proposing to build or remodel a food facility shall submit plans and specifications for review and shall receive plan approval prior to starting any new construction or remodeling. Contact DEH at 408-918-3400 or at www.ehinfo.org to obtain plan check requirements. Submit plans for the remodel by comply-by date.

    Owner unable to provide invoices for prepackaged containers of tapioca dumplings (banh bot loc) maintained inside the sliding door freezers. Facility has approximately 10 to-go container boxes maintained in the front for customer self-service and numerous in the chest freezers located in the back of the facility. The following food items were maintained inside chest freezer in the back of the facility: - Reduced-oxygen packaged marinated chicken (ga tu u muoi hoa tieu). - Reduced-oxygen packaged marinated raw pork. - To-go containers of unfertilized chicken eggs and gizzards. - Prepackaged containers of marinated pork. Owner stated that items were for personal use only. [CA] Food must be from an approved source or properly documented. [COS] Tapioca dumplings were removed from the sales floor. Items inside the chest freezer were removed from the facility.
