FOLLOW-UP INSPECTION

On-site for a follow-up inspection after the facility was closed for evidence of a rodent infestation during the routine inspection conducted on 09/24/2025 The following was provided prior to conducting the follow-up inspection: 1. The signed and completed Reopening Checklist, emailed to the assigned inspector 2. A copy of the pest control report from a licensed provider, stating that facility has been treated and that all vermin activity has been abated, emailed to the assigned inspector No evidence of rodent activity observed. Mechanical dishwasher has been repaired to output chlorine at 50 ppm. Lactic acid measured within the acceptable range of 2.11 mL/L to 4.30 mL/L in the three compartment dishwashing sink. Gap underneath the side door has been sealed. Lighting inside the walk-in cooler and walk-in freezer has been repaired. Facility reopened and may now resume normal operations.

    Per the PIC, the gas lines that are connected to cooking equipment at the main cook-line discharge unsafe levels of gas into the room (culinary lab kitchen) if the associated equipment pilot lights are not lit daily, OR if the equipment is not all manually turned off after every use. [CA] Equipment shall be maintained in good repair so that it does not pose a health concern. Fix or replace where necessary. Facility has stated that they are actively working on a fix for the issue. If any changes to electrical, gas, or plumbing is conducted, approval from the local building and/or fire enforcement agency may be required. Seek approval where applicable prior to making changes. Facility has been granted a 6 month compliance date to resolve the issue. Resolve by 04/13/2026.

ROUTINE INSPECTION

Facility is hereby closed owing to evidence of a rodent infestation. Facility is to immediately cease and desist all operations, including but not limited to food preparation, service, handling, distribution, and sale (within the kitchen area). Facility must provide the following prior to reopening: 1) A pest control report from a certified pest control agency stating that treatment was performed, and that no remaining vermin activity was observed 2) The filled out “Vermin Reopening Checklist” provided by this division Facility must also perform the following actions prior to reopening: - Eradicate all presence of vermin, dead or alive, within the facility. - Clean and sanitize all areas, including but not limited to floors, walls, ceilings, and surfaces of all equipment throughout the facility - Seal any gaps, cracks, or holes within the premises and in the structure of the facility that may provide harborage or entrance for vermin - Dispose of any contaminated items - Remove unused equipment so as to prevent harborage for vermin A follow-up inspection must be conducted prior to facility reopening. Subsequent follow-up inspections after the first shall be billed at $298/hour, one hour minimum, during normal business hours (7:30 AM to 4:30 PM), or $645 for a minimum of 2 hours, during non-business hours and in accordance with the inspector's availability.     Three compartment sink lacks any form of applicable sanitizer for the purpose of dishwashing. Per the PIC, the mechanical dishwasher is unusable given that when it is turned on, it discharges large amounts of wastewater onto the floor. [CA] A food facility must have an approved method of sanitizing food contact equipment. Food contact equipment must either be cleaned via manual wash, rinse, sanitize in the 3 compartment sink, OR ran through a mechanical dishwasher that is capable of outputting 50 ppm chlorine or water that contact utensils at 160F. Manual sanitization (in the 3 compartment sink) shall be accomplished in the final sanitizing immersion by contacting a chlorine solution of 100 PPM concentration for at least 30 seconds or by contacting a quaternary ammonium solution of 200 PPM concentration for at least 60 seconds.

The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1.
