Also to verify that no additional equipment has been added to the facility since final inspection. - A new dish washer (chlorine) has been added to the facility without approval by this department. Submit specification sheets for dish washer. - Some items were not installed per plans. Phase 2 items were left out. - Provided the following decals for facility: - Cold holding, Sanitize (wash, rinse, sanitize), hand washing, food placement.
