    Facility lacks food handler cards for staff. [CA] Each food handler shall maintain a valid food handler card for the duration of his or her employment as food handler. A valid food handler card shall be provided within 30 days of after the date of hire.

    Rice in cambros and beans in a large plastic cotnainer were measured between 45-48F. Per staff, they had been cooling since the night before. [CA] After heating or hot holding, Potentially Hazardous Foods (PHFs) shall be cooled rapidly from 135°F to 70°F within 2 hours and from 70°F to 41°F within 4 hours. Approved cooling methods include: 1) Placing the food in shallow pans 2) Separating the food into thinner or smaller portions. 3) Using rapid cooling equipment (Ex. blast chiller) 4) Using containers that facilitate heat transfer (ex. stainless steel) 5) Adding ice as an ingredient. 6) Using ice paddles 7) Using an ice bath and stirring frequently 8) Accordance with a HACCP plan. Cooling foods shall have enough space around the containers for cold air to circulate, be loosely covered, or uncovered and stirred as frequently needed to evenly cool. [COS] Items were discarded.

    Ground beef and raw carne asada were found measuring between 50-55F in the prep sink. Per staff, they removed them from the walk in cooler about an hour ago to grab portions. A permit shall be issued by this department when investigation has determined that the proposed facility and its method of operation meets the specifications of the approved plans or conforms to the requirements set forth by this department. This is a follow up inspection to verify compliance with major violations. Observations: K06: The hand sink was unobstructed and fully stocked. K07: All PHFs were in temperature. Curry was being hot held on a burner and chicken will be made to order. K08: The Rice pot was time marked appropriately. K13: No adulterated foods were observed. Salmon was found thawing outside of packaging under refrigeration. K14: Chlorine solution measured at 100 ppm. K39: Facility has provided an infrared thermometer. Ensure to provide a probe style thermometer to monitor final cook temperatures of thin meats such as chicken and measure interior temperatures of foods such as the curry. Continue to correct any and all violations.

[COS] The solution was adjusted to 100 ppm and dishes were resanitized.

    In the basement storage area, on the wire rack, a large unlabeled container was found stored. [CA] Food or food related equipment shall be stored in an approved fully enclosed food facility.

    1. Japanese curry was found out of temperature control between 106-120°F. Per staff, they removed it from the stove about an hour ago would keep it luke warm for service. 2. One piece of chicken was found being held in a shallow pan next to the charbroiler and measured at 90°F at around 3:50 pm. Per staff they made it around 12 pm. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] 1. The curry was reheated to 165 for hot holding [COS] The chicken was discarded after 4 cumulative hours from removal of temperature control.
