    Numerous food items sold on the market shelves are missing labels. Per PIC, the items came prepackaged from Selam Market at 2294 Bascom Ave, Cambell. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor.

    Single use sanitary towels were stored outside dispenser at the hand wash station. [CA] Single-use sanitary towels shall be provided in dispensers; heated-air hand drying device may be substituted for single-use towels.

    Found a live cockroach trapped inside empty hand soap dispenser at the handwash sink in the back. No food contamination observed. [CA] The premises of each food facility shall be kept free of vermin. Clean and sanitize areas of dead cockroaches. [COS] PIC killed the cockroach right away, and cleaned and refilled the soap dispenser.
