    --48 HOUR NOTICE— Our records indicate this facility is operating without a valid permit and has a delinquent account. Your balance due is $815.00. Payment must be made immediately to DEH at the address at the top of this report or via our website at www.Ehinfo.org. A facility that submits payment of the outstanding balance within 48 hours (2 business days) of this notice will be issued and mailed an operating permit by this department. Failure to comply within 48 hours will result in facility closure and additional enforcement, including but not limited to reinspection(s) and additional cost recovery fees.     Sanitizer within the 3-compartment sink was measured at 0PPM of quats at the time of inspection. [CA] Maintain chemical solution for manual warewashing at 100 ppm (1Tbs of bleach (5.25%)/gal of water) for chlorine and 200 ppm (1 tbs of quaternary ammonium (10%)/2 gal of water) for quaternary ammonium. [COS] PIC added sanitizer into the compartment; re-measured at 200PPM.

ROUTINE INSPECTION

This is the first routine inspection since the final inspection conducted on (8/14/24). On site to verify that new pieces of equipment have been added to the facility since last inspection. - Submit spec sheets for the two display refrigeration units in the front of store. Only holds chocolates. - Submit spec sheets for Under counter refrigeration and freezer chest. Both are commercial. These two pieces of equipment are replacing the reach ins. Holds ice cream and extra milks. Also the two reach ins within the dry storage area have been removed from facility.
