    Observed potentially hazardous foods stored including milk, buffalo milk, milk solution, heavy cream, and coffee creamer stored using time without proper time logging or time marking. Measured potentially hazardous food items maintained at 52F. Per operator, milk, buffalo milk, milk solution, heavy cream, and coffee creamer stored from 11:30am and is changed out in intervals of between 3-4 hours. [CA] Ensure potentially hazardous foods maintained by time are time labeled/time logged when removed from temperature control and served within 4 hours or discard. Procedures shall be available on-site for review and all time labels/time logs shall be kept on-site. [SA] Operator discarded containers of milk, buffalo milk, milk solution, heavy cream, and coffee creamer stored. Operator placed new containers on a time log.

Equipment that is broken, cracked, or in disrepair shall be repaired (when possible) or replaced. Unapproved equipment shall be approved by submitted plans through plan check. Continued use of unapproved equipment that does not properly function and/or have proper certifications will result in further enforcement including impound of equipment and additional inspection fees. An approved piece of equipment shall be presented to district inspector as a replacement for this currently unapproved piece of equipment, and plans shall be submitted to plan check for further approval. ROUTINE INSPECTION

Limited inspection conducted in conjunction with CO0158071 The following items were also discussed with PIC: - Proper manual warewashing. Handout provided. - Ensuring there is a designated handwash sink that is readily accessible at the front prep area. - The refrigeration prep unit from China is used with built-in tubes that siphons different liquids to mix drinks. Unable to verify if equipment was approved, and PIC states that nothing was changed since facility had went through initial plan-check review. Employee was able to demonstrate how the equipment is cleaned, and the tubes can be removed to be washed. Further review of facility's plans and equipment will be conducted. If equipment was changed and not previously approved by this department's Plan Check - facility shall cease the use of equipment and submit the addition/modification of equipment to this department's Plan Check.

[CA] Manual warewashing shall be accomplished by using a three-compartment sink where the utensils are first precleaned, washed with hot soapy water, rinsed with hot water, sanitized with an approved method, and air dried. Use 1 TBS of bleach (5.25%) per 1 gal. of water for chlorine solution (100 PPM). Use 1 tbs of quaternary ammonium (10%) per 2 gal. of water for quaternary ammonium (200 PPM). [COS] Guided employee and PIC on how to properly prepare the three compartment sink with chlorine sanitizer measured at 100 ppm. Employee sanitized previously washed equipment.

Ensure only approved containers of unscented chlorine sanitizing utensils and equipment are used.

    Observed a non-rigid wastewater line plumbed from the 2 door prep unit directly to the floor sink. [CA] Ensure non-rigid wastewater lines are plumbed to rigid wastewater lines. Rigid wastewater lines shall be installed with a 1" air gap from the bottom of the pipe and the top of the floor sink.

[CA] Ensure potentially hazardous foods are held cold at 41F or below to prevent bacterial growth. [SA] Operator to serve mochi within 3 hours, and yogurt, coconut jelly, and strawberry jelly within 3.5 hours for a total of 4 hours from when removed from temperature control. Cheese foam was discarded by operator.
