    Dead cockroaches were noted on sticky traps around the water heater at the time of inspection. [CA] Clean and sanitize area of dead cockroaches.

    Facility is using sticky fly traps around the facility to kill off flies. [CA] Insect control devices that are used to electrocute or stun flying insects shall be designed to retain the insect within the device and be installed away from the storage and preparation of food or food contact surfaces and linens. Obtain appropriate devices to kill off flies, talk to pest control.

    Unable to provide test trips for chlorine at the time of inspection. [CA] Testing equipment and materials shall be provided to adequately measure the applicable sanitization method used during manual or mechanical warewashing.

    Unable to provide a probe thermometer at the time of inspection. [CA] An accurate easily readable metal probe thermometer (accurate to +/- 2°F) that is designed to measure the temperature of thin masses shall be provided and readily available to accurately measure the temperatures of potentially hazardous foods.

    Under counter refrigeration next to cashier station was measured at 45*F at the time of inspection. [CA] PHFs shall be held at 41°F or below or at 135°F or above. Adjust properly or have unit serviced by contractor.

  • K14: Food contact surfaces clean, sanitized

    Knives/scissors are being stored in between the stainless steel table and prep tables at the time of inspection. [CA] Do not store food contact surfaces in those crevices since they do not get cleaned frequently. Obtain magnets or knife holders. [SA] All items were moved to the dish washer to be wash, rinse, sanitized.

  • K38: Adequate ventilation/lighting; designated areas, use

    Small deep fryer is not under the hood system and not approved by the department. [CA] There shall be a minimum of 6 inches overhang of ventilation hood over cooking equipment and equipment must be approved by the department. Remove piece of equipment and obtain approval.

FOLLOW-UP INSPECTION

This is a follow up inspection to verify that all major violations mentioned on 9/16/25 inspection report have been addressed. Major violations have been addressed and found within compliance: - K23: No rodents, insects, birds, or animals. No evidence of vermin activity at the time of inspection. Pest control report was provided by owner for review. Facility will have pest control on site two times within the month. Recommend having pest control come two times a month for 3-months and then revert to one time a month after. - Facility is allowed to re-open. Continue on working on all other violations mentioned on 9/16/25

The facility is herby closed by this department for vermin activity at the time of inspection. Subsequent follow-up inspections after the first follow-up shall be billed $298-hour, minimum one hour, during normal business hours: Monday-Friday 7:30AM to 5:00PM or $645/hour, minimum one hour, during non-business hours, and upon inspector availability.     Observed employee wash hands in prep sink at the time of inspection. [CA] Properly wash hands with soap, warm water and dry using single use paper towels as required. Wash hands within hand wash station and not other sinks.

    Large hole noted on top of the water heater at the time of inspection. [CA] Repair holes in structure and maintain in good repair to prevent pest harborage.

Supervisor Notified: Monica H. 4. Notification: The person in charge during inspection, Thanh Nguyen, has been informed that the facility must close immediately. [CA]: The premises of each food facility must be maintained free of vermin. A facility cannot operate if there is a vermin infestation that leads to contamination of food contact surfaces, packaging, utensils, food equipment, or adulteration of food. The facility is required to cease operations immediately and must remain closed until all corrective actions on the provided checklist are completed. Requirements Before Reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider.

ROUTINE INSPECTION

*NOTE* - This is the first routine inspection for the facility. On site to also verify that no changes have been made from original plans submitted into the department, final inspection was conducted on (01/10/2025) - No additional equipment has been added to the facility since final inspection. If any new equipment is added to the facility, submit into the Plan Check Department within Santa Clara County.

    Hand wash station within the prep area was blocked at the time of inspection. [CA] Hand washing facility shall be clean, unobstructed, and accessible at all times. [COS] PIC was able to removed bucket, that was blocking hand wash. Keep accessible at all times.

    Unable to provide a food manger certification at the time of inspection and some employees are missing food handler cards. [CA] Obtain a food manger certification within 60-days and food handler cards for all employees that handle food within 30-days.

    Grease build up within the fan guards noted at the time of inspection. [CA] Ventilation hood filters shall be maintained clean and good repair. Recommend cleaning hood filters more frequently.

    A consumer advisory is missing on menu with regards to raw meat being cooked within the broth upon serving. [CA] Provide consumer warning for the sale of raw or undercooked animal-derived foods. "Consumer Advisory" - Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase risk of foodborne illness especially if you have certain medical conditions.

    Cooked eggs rolls on top of the deep fryer were not being hot held at the time of inspection. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [SA] PIC would like to place the egg rolls within TPHC. Forms were provided to the facility and must be submitted into this department for approval.
