FOLLOW-UP INSPECTION

This is a follow up inspection to verify if violations mentioned on 10/8/29 inspection report have been addressed: - Permit fees have been paid in full. Receipt has been provided to specialist for review. - Red placard has been removed from facility. - Report was placed within mail box. - Facility is allowed to re-open and resume operations.

ROUTINE INSPECTION

This facility is herby closed by this department with regards to the following major violation: - Facility is operating without a valid health permit. Facility has an outstanding balance with the county. - A site vite visit was conducted on (9/23/25) with regards to a permit investigation. No response have been made from operator to specialist. - Subsequent follow-up inspections after the first follow-up shall be billed $298-hour, minimum one hour, during normal business hours: Monday-Friday 7:30AM to 5:00PM or $645/hour, minimum one hour, during non-business hours, and upon inspector availability. - Notify this department once issues have been addressed. ROUTINE INSPECTION

This is the first routine inspection since final was conducted that was conducted on (3/7/25). On site to verify that no changes have been made since final inspection. - No changes have been made since final inspection. - If facility decides to change menu. Notify this department and may have to go through plan check.
