    Facility has a can of residential insecticide. [CA] Only those insecticides, rodenticides, and other pesticides that are necessary and specifically approved for use in a food facility may be used.

    Sausage patties in the prep cooler was measured at 55F. The hotel insert was ovserfilled. Per staff, they filled the insert around 6 am. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] The portion was reduced and placed in the bottom section of the prep cooler to cool down.

    Sliced tomatoes were found cooling with plastic wrap tightly wrapped over the container. [CA] When cooling PHF's, food may be left uncovered or loosely covered to allow rapid cooling. [COS] Plastic wrap was lifted to promote rapid cooling.

    Facility is packaging orange juice and not labeling them. [CA] Food prepackaged in a food facility shall bear a label with the following: 1) common name of the food, 2) list of ingredients in descending order of predominance by weight, 3) quantity of contents, 4) name and place of business of the manufacturer, packer, or distributor. Note: Ensure to add pasteurization warning clause to juices. "WARNING: This product has not been pasteurized and therefore may contain harmful bacteria that can cause serious illness i nchildren, the elderly, and persons with weakened immune systems."

    1. Facility has several non ANSI accredited equipment. - 4x microwaves - 1x toaster oven [CA] New and/or replacement food equipment shall be certified or classified for sanitation by an American National Standards Institute (ANSI) accredited certification program (e.g. NSF, UL EPH, UL Sanitation, CSA Sanitation, or ETL Sanitation). 2. Facility's three door upright cooler is not maintaining PHFs at 41F. Ambient air was measured at 50F. Note: Cooler holds pasuerized dairy products only which measured between 43-45F. [CA] Refrigeration must be capable of maintaining PHFs at 41°F or below. 3. Facility is using cardboard to line floors, shelves of storage racks, and shelves of coolers. [CA] Discontinue the use of cardboard boxes to line shelves/flooring, use approved equipment made of materials that is easily cleanable, durable, smooth, and nonabsorbent.

FOLLOW-UP INSPECTION

Reinspection for inspection conducted on 04/01/2025. Owner, Jim, requested follow up inspection by phone and text. Reviewed pest control report dated on 4/3/2025. No sign of cockroach activity reported on pest control. Pest control report noted fruit flies. No sign of active cockroach activity at the time of inspection. Facility will continue monthly pest control service in the future. Facility is re-opened by this office today. OK to operate. Continue to correct violations dated on 4/01/2024 report.

ROUTINE INSPECTION

Facility is closed by this office due to cockroach infestation. Facility will remain closed at this time and will not reopen until a follow up inspection is conducted. Facility is to cease and desist all food sales and operations. The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. Failure to comply may result in enforcement actions per County Ordinance Code Section B11-55. A follow up inspection will be conducted. The first follow-up inspection is free of charge. Subsequent follow up inspections after first follow up shall be billed $298/hr during business hours; $645/hr during non business hours (after 4:30pm Monday through Friday, county holidays and weekends) upon availability. Failure to comply may result in enforcement action. Discussed TPHC for milk self service area with PIC. PIC will use timer and discard milk in 4 hours. No active warewash observed at the time of inspection. Chlorine uses as sanitizer. Test strip available. Report email to operator due to printer malfunction. PIC confirmed receipt of email.

Last pest control report was Dec 2024. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately.
