    Observed ice cream scoop handle in direct contact with ice cream. [CA] Position scoop with handle sticking up so that it is not in contact with food.

    Observed potentially hazardous foods that measured in the temperature danger zone located in the food prep cold hold unit (cooked potatoes 46F). Per staff, the potatoes were portioned today. [CA] Ensure PHFs are properly cold held at 41F or below OR hot held at 135F or above. Repair, adjust or replace units if necessary. Minimize the amount of time PHFs are in the temperature danger zone; recommend placing PHFs back in reach-in cooler to quickly cool back down to 41F or below.

Apply by 5/29/25 or remove equipment from facility.

OWNERSHIP CHANGE INFORMATION: NEW OWNER: Mojo Hill Cafe, LLC FACILITY NAME: GVA Cafe The applicant has completed the change of ownership application process for an Environmental Health Permit. The permit category for this facility is FP10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 02/01/2025 - 01/31/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate. New ownership start of operation date: 02/01/2025 Post permit in public view. Structural Review inspection conducted on 01/24/2025. Prior to the installation of any new equipment, submittal is required to Plan Check. Plan check consultation hours are Monday - Friday 730AM-10AM and afternoon by appointment only at address above.
