    Lack of a device to hang mops, all the mops and brooms are sitting in the mop sink. [CA] Mop rack or other devices not made available to hang and store cleaning tools. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

    Measured cheese in hot holding unit at 115F. Per employee it was stored in the unit around 10 AM. Measured shell eggs at 60F sitting on prep table. Per PIC, eggs were taken of out of a refrigerator less than an hour ago. [CA] PHFs shall be held at 41°F or below or at 135°F or above. [COS] Melted cheese was discarded in trash.     Observed food preparation sink waste pipe extending into the floor sink. [CA] All equipment that discharges liquid waste discharge through an airgap of at least one inch into a floor sink or other approved type of receptor.

    Observed one live cockroach coming out from under the two door refrigerator in cook line area. No other live or dead activity observed in the facility. [CA] The premises of each food facility shall be kept free of vermin. A food facility shall not operate when there is a vermin infestation that has resulted in the contamination of food contact surfaces, food packaging, utensils, food equipment, or adulteration of food(s). The food facility shall cease operation of the food facility immediately. The food facility shall remain closed until: there is no longer evidence of a vermin infestation; all contaminated surfaces have been cleaned and sanitized; and contributing factors such as cleaning, repairs, and the elimination of harborages have been resolved. [CA] PIC killed the cockroach and cleaned the area. Review pest control report from January. Per PIC, another service is scheduled for next week.
