Follow-up for facility closed due to rodent infestation. At time of inspection: - No rodent droppings or other signs of a rodent infestation was observed. - All affected areas noted on previous inspection has been cleaned and sanitized. - Observed gaps and openings in walls have been sealed. Operator will continue to eliminate and seal any additional gaps, openings, or crevices that they find. - Reviewed pest control reports dated: 7/4/25, 7/5/25, 7/6/25, and 7/7/25. ROUTINE INSPECTION

Limited inspection conducted along with complaint investigation. Facility shall cease and deist all food operations. Facility is closed due to active rodent infestation. Observed rodent droppings in back warehouse, deli and bakery area. Pest control service is conducted weekly. Pest control service comments/observations such as open holes/gaps, standing water and food debris in kitchen area and removing rodent carcasses from traps and replacing traps/glue boards also indicate active infestation. Food facility shall remain closed and complete the following requirements before reopening: 1. Email the signed and completed Reopening Checklist to the assigned inspector. 2. Submit a copy of the pest control report from a licensed provider Contact district specialist Sukhreet Kaur at (408) 918-3460 or sukhreet.kaur@deh.sccgov.org to schedule re inspection once the above requirements are completed. - Subsequent follow-up inspection after first follow-up shall be billed $298/hour, minimum one hour, during normal business hours (Monday - Friday, 7:30 AM to 4:30 PM), and $645/hour, minimum two hours, during non-business hours, and upon inspector availability. - The posted placard is property of the Santa Clara County Department of Environmental Health and shall not be removed, covered-up, relocated, tampered, or copied. [CA]: The premises of each food facility must be maintained free of vermin. The juncture of the floor and wall shall be coved with a 3/8-inch minimum radius coving and shall extend up the wall at least 4 inches.
