FOLLOW-UP INSPECTION

Follow up inspection for routine inspection on 02/27/2025 regarding evidence of dead cockroaches. During the inspection, Specialist found no evidence of cockroaches at the facility. Per Manager, Terminex came today to conduct a pest control service. Service report provided and noted German cockroach activity. Manager stated that the facility shall now receive monthly pest control services from Terminex. Ensure continued monitor activity for vermin and to follow pest control company recommendations. Maintain food and non-food contact surfaces clean and free of debris. Discussed ensuring all entry doors are maintained closed to prevent entrance of vermin.

OWNERSHIP CHANGE INFORMATION NEW FACILITY NAME: RAW SUPERFOOD CAFE NEW OWNER: 39 RAW SAN JOSE, LLC The applicant has completed the facility evaluation application process for an Environmental Health Permit. The permit category for this facility is FP 10. An invoice for the permit fee in the amount of $1,107.00 will be mailed to the billing address on the application. A 25% penalty fee shall also apply in addition to the $1,107.00 permit fee for operating without a valid health permit as stated on routine inspection conducted on 02/17/2025. Payment must be submitted within 10 days of receipt of the invoice. The owner is responsible for contacting our department if an invoice is not received and remit payment within 30 days. The Environmental Health Permit will be effective: 03/01/2025 - 02/28/2026. This report serves as a temporary permit. However, the permit will be deemed invalid if the permit fee is not paid in full within 30 days from the date of this report. Okay to Operate.
