PIC provided invoice for the following: ziploc bag siu mai Ziploc bag Shrimp dumpling 6 packs of dry shrimp Per PIC, Dua Sau Rieng Icecream is made on site and it was pre-approved by previous inspector from office. Yogurt is not part of the approved item. Other items were purchased from locations such as Duc Huong. PIC stated that she did not obtain receipts when purchased. Food is removed from sales floor and take home. Continue to correct violations dated on 11/20/2024 report.

    Frozen items did not have label on containers. Per employee, tried to call owner but owner did not pick up the phone. Employee did not know where are the frozen food from. The two freezers were impound at this time. See impound report for detail. [CA] Food must be from an approved source or properly documented. [SA] Freezers were impound. Provide invoice for food source within 30 days.

    Containers of candies were stored on the floor under 3 compartment sink next to chemical. [CA] Food shall be stored at least 6 inches above the floor to prevent contamination. Store chemical away from food.
