Soda machine installed without submitting plans to and obtaining approval from DEH Plan Check. Soda machine's drain line observed to be directly plumbed. PIC stated unit was installed this month. *REPEAT VIOLATION* Missing valid food safety manager certificate. The food safety manager certificate on site is from an employee who has not worked at the facility for approximately 60 days per PIC. [CA]: Provide proof of a valid food safety manager certification. Food facilities that prepare, handle, or serve non-prepackaged Potentially Hazardous Food shall have a valid food safety manager certificate available on site for review at all times.

At the back hand sink near the office, dispenser was not providing single use towels. [CA]: Hand sinks shall be equipped with single use towels at all times.
